The family-history pages and contribution form are still being prepared. The contribution form is not open yet. This notice describes the planned contribution process and the form\u2019s current settings.<\/p>\n\n\n\n
The prepared form accepts text only. It asks for the person, employee or family the contribution concerns, the contribution type, a story or description, permission instructions, an attribution choice and an acknowledgement of the review process.<\/p>\n\n\n\n
Your name, contact email and source details are optional. The form does not accept photographs, documents or other file uploads.<\/p>\n\n\n\n
Please leave out Social Security numbers, financial information, private addresses, medical details and personal information about children. Include only what is needed for the family-history contribution.<\/p>\n\n\n\n
Contributions are intended for review before anything is added to a public page. Submitting the form will not publish a contribution automatically.<\/p>\n\n\n\n
The form offers three permission choices: review only, permission to publish material you own after review, or contact before publication. It also lets you keep your name private, request credit using the name you supply, or ask to be contacted about attribution.<\/p>\n\n\n\n
A contact email is for communication about your contribution and will not be included in a published story. Permission to share a contribution does not cover unrelated material or someone else\u2019s private information.<\/p>\n\n\n\n
The form is currently configured to store submissions in the website\u2019s database for administrator review. This includes the information entered in the form and the permission and attribution choices.<\/p>\n\n\n\n
The form is configured to delete stored submissions after 365 days (one year). Forminator\u2019s site-wide IP-address retention setting is configured to delete stored IP addresses after 7 days. Permission records for published stories must be preserved separately before the related form submission is deleted. An account-erasure request does not automatically remove a form submission under the current settings.<\/p>\n\n\n\n
The prepared form has no automatic email notifications or connected third-party app integrations. These settings do not establish that a submission is anonymous or that the website and its hosting services keep no technical logs.<\/p>\n\n\n\n
There is no visitor file-upload option in the prepared form. Photographs and documents are considered individually for permission to display, identification and privacy.<\/p>\n\n\n\n
Identifying an item\u2019s source does not by itself establish permission to republish it. Any future upload option will need its own review of storage, access and privacy arrangements before it opens.<\/p>\n\n\n\n
Some prepared family-history pages display map images supplied by Wikimedia and link to historical records on other websites. Loading an externally hosted image connects your browser to that provider. Following a source link takes you to another website, which has its own privacy practices.<\/p>\n\n\n\n
